The Rita Carlson Service Award

This award was founded in 2024 as part of our 20-year celebration to thank Rita Carlson for over 25 years of dedication to the Health Ministries Network of Texas.
 

This award is designed to pay special tribute to those individuals who, through
distinguished service to their faith organization, local communities, and or service to those in need are making a difference in health via spiritual care.

 

2024 Award Recipient: Rita Carlson

It is with a great honor that Health Ministries Network of Texas Announces the Rita Carlson Service Award.

Rita Carlson received the award, named after her, as our very first recipient. She has provided over 25 years of service to this organization. She was honored at the Texas State Capital, at the HMNT Network Meeting.

We are so excited, as your state organization, to offer a State Award to those who are faithfully serving in Health Ministry.

Nomination Process

1. Each Nominee must meet all of the award eligibility requirement(s) to be considered.

2. Current members of the HMNT Board of Directors, Leadership Committee, Awards Committee or staff may not be nominated for any HMNT award, and shall not nominate, write letters of support, or endorse an individual for an HMNT award.

3. Letters of support and other forms of validations should accurately describe how each of the nominee’s professional achievements or contributions meet the specific aware selection criteria.

4. Nominators might be required to verify the accuracy of the information included in the nomination
 

Nomination Submissions

1. A complete award nomination consists of the following:

a. Submission of all required information, for all required fields

b. A copy of the nominees most recent Curriculum vitae (CV) If a CV is not available, then should include as much information on the nominee’s education, work experience and professional accomplishments as is

2. Requirement:

a. Membership is not a requirement to receive an award, however the award must be submitted by an HMNT member in good standing.

b. Award is open to any individual, both RN and Non-RN for their contribution and service to the community

c. Have served a minimum of 5 years in their current community/volunteer role
